Pre-K Online Application
Pre-K applications can be submitted online for the 2020-21 school year.  Residency: Must live in Leavenworth USD 453 district and be able to provide proof of residency.

Age: Preschool student must be 4 on or before August 31, 2020

Program Criteria:
• Parent younger than 20 when student was born
• Single parent
• Parent(s) NOT obtaining a high school diploma or GED
• Eligible for free lunch (you must fill out a Child Nutrition application after July 1, 2020, in order to verify this qualification)
• Referred by SRS
• Limited English Proficiency
• Migrant student
• Developmentally, or academically delayed, based on valid assessment

If you have qualified for at least one of the criteria listed above, you can complete and submit the
Preschool application electronically (https://bit.ly/453PreKapplication) OR return a hard copy to the Leavenworth Board of Education Office at 200 N. 4th Street, Leavenworth, KS. Your application will be kept on file and you will be notified by telephone within 2 weeks of school starting regarding the status of your application. It is crucial that if your telephone number changes that you contact the Board of Education Office (913-684-1400) so your application can be updated.

When you receive your telephone call stating that your child has been accepted into the program, you will need to bring the following documents to the school building in order to complete the enrollment process:
• Birth certificate (we prefer the certificate of live birth vs. the hospital copy)
• Immunizations (all immunizations MUST be up-to-date)
• Proof of residency (utility bill or lease agreement)
• Physical
